Ask City staff which application and security apply to your ROW operation rather than choosing a listed trade form as a substitute.

Describe the exact location, opening/installation method, affected public space, responsible contractor and restoration requested for that one job. Obtain the accepted legal principal/recipient, bond form, amount, effective dates and covered permits; do not assume a continuing blanket instrument covers it or that a single-job purchase covers another site.
