Indiana — DOT Blanket Highway Permit - Broadband - MAIL Bond

Match INDOT broadband blanket security to the fiber-owning company and covered permits. The Broadband Guidelines currently linked by INDOT state a $500,000 blanket minimum and require capacity for all open permits; a smaller single-permit performance figure is not that blanket minimum.

What this bond does

Multiple-permit State ROW security for the actual INDOT Broadband Access request, using the accepted blanket form and capacity.

Who needs it
The broadband company required by INDOT's request. Its linked guidelines place the surety in the fiber-owning company's name and identify the installing subcontractor separately.
Who it protects
The current State blanket form conditions permitted-work performance and lawful contractor/vendor/labor claims for covered permits. It uses aggregate security with a specified court-cost/attorney-fee exception, not a fresh full bond amount for every permit or every private project.

Your bond option

This is the option on record for this bond.

The bond amount describes the limit of the bond obligation, not the price. A listed amount does not establish what the requesting party requires. The premium is what you pay and must be confirmed for the actual option. An E&O limit, where listed, is separate errors and omissions insurance.

Identify the fiber owner and actual State activity

INDOT's current program distinguishes Broadband Access from other occupancy, driveway and miscellaneous uses. Its linked Broadband Guidelines, updated August 2018, require the surety in the company name that owns the fiber and separately identify the installer/subcontractor. Describe the route, start/end points, purpose and installation quantities; do not automatically name the boring subcontractor as principal.

The guidelines also address an executed Broadband Corridor Agreement for their specified corridor/facility situations. Confirm whether that separate agreement applies to your location. Purchasing a blanket bond does not create corridor access or replace the actual Broadband Access permits.

Keep sufficient blanket capacity across open permits

The currently linked guidelines state a $500,000 blanket minimum and require the broadband company to keep the combined obligations of active permits within its surety capacity. If exceeded, no new permits are issued until security increases or permits close. Confirm the current requested amount and covered types with INDOT; do not use the guidelines' separate single-permit performance amounts as blanket security.

The current three-page Blanket Bond has Broadband Access and other permit-type checkboxes. It covers the named principal's permitted work under the accepted categories. Match those categories and all outstanding permits rather than assuming the provider's broadband label creates unlimited work authorization.

Complete both EPS and original-document filing

Purchase Now opens Propeller; the option displays no fixed amount or term. Confirm the required sum, principal/State recipient and actual effective date. Complete all three current form pages, including both notarized signature pages and the surety power of attorney. INDOT's current resources require an EPS PDF attachment and signed/notarized hard copy, with blanket originals directed to its central Permits office.

MAIL remains part of the offering's label; it is not a promise that the provider completes INDOT filing. The State form takes effect at the later of its stated effective date and INDOT acceptance/EPS entry, and continues until released by the State. Track actual permits and use INDOT's published release-request process rather than assuming one completed installation ends the blanket bond.

Questions about this bond

Can I use a single-permit performance minimum for this blanket?

No. The currently linked August 2018 Broadband Guidelines separately state a $500,000 blanket minimum and require capacity for open-permit obligations. Confirm INDOT's current request rather than borrowing the smaller performance figures.

Should the installer automatically be the principal?

The linked guidelines place broadband surety in the fiber-owning company's name and identify a subcontractor separately. Match the actual INDOT applicant and ownership request.

Does MAIL mean an EPS attachment is unnecessary?

No. Current INDOT guidance requires a PDF in EPS and signed/notarized originals. Confirm which delivery steps the issuer handles; the label does not establish State filing or work permission.
