Indiana — DOT Blanket Highway Permit - Driveway - MAIL Bond

Use this INDOT driveway blanket offering when the State's request calls for security covering multiple permits for the same principal. A driveway access permit, an owner's consultant consent and the blanket bond are separate documents.

What this bond does

Multiple-permit security for the named principal's approved INDOT driveway/road-approach work in State ROW.

Who needs it
The legal permit principal required by INDOT, coordinated with the property owner and any consultant submitting the driveway application.
Who it protects
The State blanket form addresses performance and lawful payment claims associated with covered permits. Aggregate security is shared across those permits, with the form's specified cost/fee exception; it is not a fresh limit for each private development.

Your bond option

This is the option on record for this bond.

The bond amount describes the limit of the bond obligation, not the price. A listed amount does not establish what the requesting party requires. The premium is what you pay and must be confirmed for the actual option. An E&O limit, where listed, is separate errors and omissions insurance.

Identify the access and party receiving the permits

INDOT's driveway program connects private property to State-controlled roads and distinguishes private/field access, temporary driveways, commercial approaches, removals and public road approaches. Give INDOT the actual property, road and proposed change. A county or municipal driveway permission is not this State request.

When a consultant submits through EPS for a property owner, INDOT lists a separate Consultant Consent Form. It also lists an access-control agreement that may be required for the actual driveway approval. Those documents do not replace the bond or automatically make the design consultant its principal.

Confirm blanket rather than single-permit security

INDOT distinguishes blanket bonds covering multiple permits from performance bonds covering one permit. Confirm which request you received before using a blanket offering for a single approach. On the current three-page Blanket Bond, identify the legal principal, State recipient, required amount/date and approved Driveway category, with any other categories actually accepted by INDOT.

The form applies an aggregate amount to covered permits, including additional permits issued later. It becomes effective at the later of the stated date and INDOT acceptance/EPS entry, and stays in force until State release. A permit review estimate, one driveway's completion or the purchase anniversary does not establish expiration.

Arrange issuance, both filing steps and access approval

Purchase Now opens Propeller; the option displays no fixed amount or term. Obtain INDOT's actual blanket sum and accepted execution instructions; do not borrow a broadband minimum or treat a generic permit-bond definition as a separate driveway blanket schedule.

The current form needs all three pages, notarized principal and surety signatures and the surety power of attorney. Current resources require an EPS PDF and original signed/notarized documents; blanket originals go to the central Permits office rather than the district route used for single-permit performance bonds. Confirm the provider's MAIL arrangements, track State acceptance and obtain each actual driveway permit before work. Use the published bond-release process when appropriate.

Questions about this bond

Does one driveway automatically require a blanket bond?

INDOT distinguishes blanket security for multiple permits from performance security for a single permit. Confirm the actual request before selecting this blanket offering.

Is consultant consent the same as bond filing?

No. INDOT lists consent for a consultant applying on the property owner's behalf. Match the legal permit principal separately and complete the actual security filing.

Does MAIL mean the permit can be submitted only on paper?

No. INDOT accepts permit applications through EPS and requires both the bond PDF and signed/notarized originals. Confirm what the provider sends and what you must submit; neither checkout nor mailing grants access.
